Introduction : The rise of nosocomial infections is considered as a significant health threat, according to longer hospital stays and higher mortality rates. The increasing resistance of microbes to antibiotics ensnare treatment, making difficult to select effective medications. This study aims to investigate the antibiotic resistance patterns of common pathogens in nosocomial infections in Alavi Hospital, and emphasize the significant role of resistance pattern to manage and control these infections. Methods: In this cross-sectional study, we collected data on antibiotic susceptibility (antibiograms) from patients with four common types of hospital-acquired infections from september2023 until September 2024. Out of 540 samples, 178 with positive cultures were detected and their resistance pattern was analyzed according to CLSI standards. Results: The analysis showed that 40% of positive culture samples were Escherichia coli (E. coli), with 84.53% resistant to third- and fourth-generation cephalosporins. 26.4% of cases were Klebsiella , showing 94.44% resistance to beta-lactamase inhibitors. Pseudomonas aeruginosa made up 14% of cases, with 79.16% resistance to carbapenems. Acinetobacter was found in 10% of cases, exhibiting 100% resistance to carbapenems and 93.75% resistance to fluoroquinolones. Staphylococcus aureus accounted for 6% of infections, with 87.5% resistance to beta-lactams. And 2.2% of the samples were Enterococcus, showing 100% resistance to clindamycin. Conclusion: The growing prevalence of antimicrobial resistance in comparison with past years, specially Gram-negative bacteria, affirms the urgent need for strict antibiotic stewardship programs. The resistance patterns observed in this study highlight the increasing presence of multi-drug resistant pathogens, emphasizing the critical importance of infection control guidelines such as hand hygiene, environmental cleaning , and patient isolation to reduce nosocomial infections and control antibiotic resistance.
