Objectives: Stenotrophomonas maltophilia (S. maltophilia) is an emerging multidrug-resistant global opportunistic pathogen associated with high mortality. In this study, we evaluated the antibiotic-resistance profile and antibiotic resistance genes among S. maltophilia isolates. Materials and Methods: S. maltophilia isolates were collected between 2021 and 2022 from four tertiary care hospitals in Iran. Antimicrobial susceptibility was evaluated by Kirby–Bauer disc diffusion method. polymerase chain reaction (PCR) was used to determine the presence of sulfonamide resistance genes. Results: Among the 94 collected S. maltophilia isolates, the highest susceptibility was against trimethoprim-sulfamethoxazole (96.8%) followed by minocycline (95.7%) and levofloxacin (93.6%). Furthermore, sul1 and sul2 genes were detected in 61 (64.8%) and 31 (32.9%) isolates, respectively. Conclusion: The present study reveals a low frequency of antibiotic resistance among the S. maltophilia isolate, while the prevalence of antibiotic resistance genes was high. Thus, identification of these genes for infection control programs and to prevent the spread of resistant bacteria is very important.
