Background: As the causative agent of tuberculosis (TB), Mycobacterium tuberculosis(M. tuberculosis) could live in its host' body without causing any complications; therefore, many people worldwide are asymptomatically infected with this bacterium. Due to its contagious nature, the Coronavirus disease 2019(COVID-19) pandemic caused several respiratory illnesses like TB to be neglected or less noticed by the health systems of various countries. Thus, the present research aimed to evaluate the effect of this pandemic on TB disease in western Iran. Methods: This before-after study was performed on 403 TB patients in Hamadan province (western Iran) from 2017 to 2022. The patients' medical records, 255 cases before and 148 cases during the pandemic, were collected and compared. SPSS software V.26 was used for data analysis by employing Chi-square and Fisher's exact tests, logistic regression modeling, and independent samples t-test. Results: The average age of TB patients was 57.3 ± 21.8 years. Among the 148 TB patients during the pandemic, molecular diagnostic test was performed for only 57 patients, of whom only 14 (24.6%) had a positive molecular test for COVID-19, and 43 (75.4%) were negative. The results showed that before the pandemic, resistance to isoniazid was zero, but after the pandemic, isoniazid resistance was observed in seven (14.3%) patients; however, no statistically significant variation was observed in this regard (p= .099). The findings of logistic regression modelling showed that the most significant variable associated with treatment success was the patient's age so that the chance of recovery was significantly higher in younger patients (p= .007). Conclusion: The findings indicate that the COVID-19 crisis has affected other respiratory illnesses (like TB) and caused an increase in antibiotic resistance and the prevalence of non-tuberculous diseases and a decrease in the recovery of TB patients. Therefore, it is suggested to run a rapid and efficient response simultaneously to both TB and COVID-19 in terms of careful monitoring and provision of appropriate health care services.
